Available 24 hours a day, seven days a week. A EAP representative will assist you and, if necessary, refer you to a qualified licensed professional for a face-to-face consultation. Prescription Drug Program All employees who elect to have group medical coverage will receive the Pharmacy Benefit Program as well. The three-tier design covers nearly any prescription medication, and it makes your out-of-pocket expenses predictable and easier to manage on lower cost drugs. Getting prescriptions filled is easy when you present your medical identification card to a participating pharmacist. Generic drugs cost only $10.00!A home delivery program is also available, where you can save by getting up to a 90-day medication supply for the cost of a smaller supply, and your prescription is delivered directly to your home. The program is easy to use and you’ll receive simple, step-by-step instructions once you are enrolled.Life Insurance and AD&D RMI provides full-time active employees with a basic group life insurance policy and accidental death and dismemberment (AD&D) insurance at a rate of one times their annual salary to a maximum of $50,000 and a minimum benefit of $10,000 (rounded to the next higher multiple of $1,000). RMI pays the full cost of this benefit. Employees who work 30 hours or more are eligible after 90 days of employment.Tax Deferred Retirement Account (401K Plan) All employees are eligible to participate in the 401K plan after completing 60 days of employment. You may join, add, or delete your deferred contributions on a quarterly basis. RMI will match up to 2% of the funds you defer into your 401K. For every dollar you defer from your salary into your 401K, RMI will match 33%, up to a total of 2% of your annual income. For example, if you defer 6% each month, RMI will match 2%, if you defer 3%, RMI will match 1%, if you defer 1%, RMI will match .33%. If your annual income is $30,000, and you defer 6%, or $1,800 per year, RMI will contribute $600 towards your plan. You may defer up to 100% (or IRS maximum) of your annual compensation at a maximum contribution of $15,500 for those under 50 years old, and $20,500 for those 50 and older. You must have worked a minimum of 1000 hours in a calendar year and be employed with RMI on the last day of the calendar year, to be eligible for the matching funds. You are automatically vested in the funds you defer; however, the matching contributions have a gradual vesting schedule. Please refer to the 401K-plan document for more details. VisionEmployees who select medical insurance also have the benefit of the Routine Vision Program. DentalRMI’s plan allows you to seek treatment from the dentist of your choice, with both in and out of network options, with our flexible PPO plan. Preventive Services like oral exams, cleanings, fluoride, sealants, x-rays, space maintainers are covered at 100% when using an in-network provider!Pre-Paid Legal Program and Identity Theft Shield RMI offers all full-time and part-time employees the benefit of Pre-Paid Legal Insurance. This inexpensive insurance allows RMI employees to feel comforted with coverage that assists them in legal needs they may have come up at a moments notice. With this coverage, the employee’s benefits grow as they stay with the plan from year to year. Under this plan, employees can create their own Will at no charge to the employee and only a charge of $20 for their spouse. That is a savings of over $400. Employees can also get legal advice, document review, motor vehicle legal services, trial defense services, IRS audit legal services, preventative legal services and court time with the assistance of a lawyer assigned to the employee from a local law firm. There is also an option of identity theft protection program. FSA Medical and Dependent CareRehab Management, Inc. provides you the opportunity to pay for out-of-pocket medical, dental, vision and dependent care expenses with pre-tax dollars through the Flexible Spending Account. You can save approximately 28% of each dollar spent on these expenses when you participate in the FSA. Health Reimbursement Account expenses are limited to $5000.00 per plan year: Dependent Care Reimbursement Account expenses are limited to $5000.00 per plan year. The biggest benefit of using our health insurance carrier for FSA is that this allows you to automatically file your reimbursements for any and all medical charges you incur. This includes co-pays and prescriptions. You will still need to file reimbursement claims for those outside of medical (dental and dependent care, for example). Remember this is a “use it or lose it” program. Please note that you may also include mileage to and from doctor appointments as well as tolls and parking. Long-Term DisabilityLong Term Disability is given to all full-time employees and is paid 100% by RMI. Based on the benefit, employees can use their disability insurance after a 90-day elimination period and covered at a rate of 60% of their income until the age of 65, to a maximum benefit of $5,000 with a minimum benefit of $100 or 10% of benefit based on monthly income loss before the deduction of other income benefits. For as long as you remain totally disabled, LTD benefit payments will continue according to the duration schedule. See LTD Literature for more details.Disability Income Benefits RMI provides full-time employees with Extended Illness Days. EID is a benefit that employees can use for medical reasons only. There is a five-day waiting period to use this benefit, in which the employee can use Paid Time Off or unpaid days (if all PTO has been exhausted) for the waiting period. With a doctor’s note, the employee can start using their EID time and continue until the employee’s EID is exhausted or the employee returns to work. Just as the employee needs a doctor’s note to go on leave, they must also have a doctor’s note to return to ensure the employees ability to work at their normal work schedule and job duties. EID balances can be found on the right side of your pay stub. Full-time employees are eligible 90 days after their full-time status begins and they begin accruing immediately. There is a maximum of 65 days accrual with carryover. All 40 hour/week employees accrue 4 hours per month. All 30 hour/week employees accrue 3 hours per month Paid Time Off Paid leave for vacation and holidays earned is based on the number of hours an employee works in a week, the job and length of service. Vacation time, sick leave and holidays are included in one accrual. PTO will begin to accrue immediately however employees are not eligible to use this benefit until after 90 days of employment. Any days off during the first 90 days of employment must be pre-approved before hire and will be unpaid. PTO can be used in 1-hour increments. All requests for PTO of more than one week must be submitted 30 days in advance. All requests for PTO of less than one week must be submitted one week in advance. Maximum PTO carryover is 120 hours per calendar year. Birthday Paid Day Off Our employees asked for it and RMI heard them! We added an extra day of time off for your Birthday in 2005. This benefit is for all full-time/part-time employees who have been here for a year or more. You will receive a voucher with your birthday card. All eligible employees will have one year from their birthday to take their special day. Unlike the PTO time, employees will not accrue this birthday day, but will be available to them starting one year after their hire day and beginning on their birthday! Worker’s CompentationProvides for medical expenses and/or loss of income from injuries or illness determined to be work related. Paid by RMI and all employees are eligible.Unemployment Insurance Provides unemployment income in accordance with federal and state law. Paid by RMI and applies to all employees who are eligible in accordance with applicable state laws. Professional Liability InsuranceRMI provides all therapists and assistant therapists with $1,000,000.00 per incident malpractice/liability insurance. Supplemental Individual Policy OptionsRMI employees also have the option of participating in multiple voluntary insurance and business products. This program provides individuals with access to insured and non-insured health care and business products offered on a voluntary basis. Listed below are the types of insurance and business products available to you. Individual Medical, High-Deductible Plans, Health Savings Account, Co-pay Plans, Saver Plans, Select PPO Plans, “Any Doc” PPO Plans, Managed Indemnity Plan, Limited Medical, Rx/Health Discount Card, Short-Term Medical, Dental, International/Travel Medical and Term Life Insurance.Social SecurityRetirement income, disability income and Medicare benefits in accordance with Social Security laws. Direct Deposit We strongly encourage Direct Deposit, which eliminates unnecessary trips to the bank or waiting in lines, a more timely, safe and secure method of receiving your pay. You may have up to three accounts at one time. You will still receive a pay stub in the mail. It takes less than two pay periods to go into effect. Jury Duty LeaveProvides for regular pay, minus any jury duty pay received for any hours missed from work. All full-time employees who work 30 hours or more are eligible after 90 days of full-time employment. Bereavement Leave RMI provides 2-5 days of leave that does not count against your PTO, in the event of the death of an immediate family member. All full-time employees working 30 hours or more are eligible for this benefit on day one of full-time employment. Military Leave Provides for a medical leave of absence to be granted for any employee called to active duty with the US Armed Forces. All Employees are eligible. CEU FundingRMI provides funding for employees to participate in continuing education courses. The amount of this benefit is based on the number of hours an employee works per week and job title. Employees are eligible after 90 days of full-time employment. Courses must be related to employee’s job description and be approved by Regional Manger (or direct supervisor, if not a Therapist or Therapist Assistant.)Professional Dues and Annual LicensureAfter one year of employment, all full-time clinical employees will receive $100 annually to be used to pay their annual association dues or to renew their licensure. If RMI asks you to become licensed in more than one state, RMI will pay for that additional state licensing. New Graduate Program RMI provides 24 months of mentoring/coaching, goal setting, and growth planning for all therapist and therapist assistants with less than 2 years of experience. Royal Flush Employee Recognition ProgramEmployees are recognized for various accomplishments either by their peers or by management and are awarded points, which may be exchanged for items in our Royal Flush Store. Point totals will be forwarded to staff on a monthly basis with your available balance. Please recognize your fellow employees for points toward Royal Flush items. Annual Holiday Party RMI hosts an annual end of the year holiday party to provide employees a chance to celebrate the years of accomplishments in a festive and fun manner.
